Required Athletic Forms
All WSCA athletes are required to read, complete, and turn in the following forms to the coaching staff before beginning any school sports activities, including practices. Contact your coach if you have any questions.
Please print, complete and return the pre-participation physical to the front office or to the coaching staff. All other forms are viewed here and acknowledged in PowerSchool portal. Both parent and student must complete the form in PowerSchool.

Pre-participation Physical Evaluation (High School only) -Submit to the coach. We cannot accept physicals from a chiropractic office as they are not CIF approved
